Audley Striders Walking Group – MEMBERSHIP FORM

In order to become a member of Audley Striders Walking Group all members are asked to complete the brief form below.  To ensure the safety of all our members the form asks for your contact information and emergency contact details of your next of kin.  Please also let us know if you have any medical conditions that you think we should be aware of. Please note the form also asks you to upload a clear photograph of yourself.  This is due to the growing number of members we now have, and again is for safety purposes. This image will be attached to your membership record on our system. Please have the photo saved on your device, ready to upload before commencing to fill in the form.

All information received will be retained in line with GDPR (Data Protection Act 2018) and will be kept confidential and Club Leaders will only refer to your information, should an emergency arise either whilst walking with Audley Striders Walking Group or taking part in related activities, such as fitness classes.
